Occupational Therapist, Regional Geriatric Program of Southwestern Ontario - 1 position

Temporary Full Time
Salary Range: $40.36 - $56.30 /hour


This is a temporary full-time position that is expected to extend until March 31, 2027, and is subject to the availability of work

The focus of this work will be community-based comprehensive falls assessment.

The Occupational Therapist (OT) will work in partnership with Middlesex-London Community Paramedicine (CP) team to provide in-home clinical assessment to older adults experiencing falls in the community. They will facilitate access to specialized geriatric services through consultation with a geriatrician to develop a comprehensive care plan focused on community-based intervention to decrease falls risk and improve mobility.

As a member of the project team engaged in developing and testing this innovative model of care, the successful candidate will work closely with the Regional Geriatric Program of Southwestern Ontario Research & Strategy team to participate in aspects of the quality improvement process including clinical model evaluation.



Essential Qualifications

  • Current registration with the College of Occupational Therapists of Ontario
  • Demonstrated knowledge base in the provision of service to the geriatric population
  • Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
  • Excellent communication skills; both written and verbal
  • Excellent organizational, time management, critical thinking and problem-solving skills
  • Must be capable of establishing positive interpersonal relationships and able to work as an effective team member
  • Demonstrated experience using a patient-centered approach
  • Self-directed and self-motivated to accomplish specified outcomes
  • Demonstrated commitment to evidence-based practice
  • Knowledge of and ability to mobilize community resources
  • Experience in the development of teaching materials and the teaching of clients, family, and caregivers
  • Working knowledge and experience with theories and principles of guiding adult learning and patient self-management approaches
  • Proven flexibility and a high level of initiative
  • Familiarity with outcome measurement related to clinical practice
  • Flexibility in adapting to varying caseload needs
  • Knowledge of computer systems (e.g. workload measurement, Cerner, GroupWise, Microsoft Office)
Preferred Qualifications
  • Proficiency in French would be an asset
  • Demonstrated knowledge base in the provision of services to patients requiring geriatric rehabilitation, musculoskeletal rehabilitation and who may also have geriatric mental health issues
  • Minimum of 2 years full-time related experience as an Occupational Therapist
  • Assistive Devices Program (ADP) Authorizer status
  • Experience in both hospital and community service delivery
  • Demonstrated interest and/or experience in research communication and program evaluation

Immunization Requirements
  • Provide vaccination records or proof of immunity against measles, mumps rubella, varicella (chicken pox), Hepatitis B, COVID-19 and influenza.
  • Provide documentation of the Tuberculosis skin testing
