The St. Joseph's Family Medical Centre was the first community-based Family Medical Centre in Canada. It was founded in 1969 as a partnership with the University of Western Ontario, St. Joseph's Hospital, and the College of Family Physicians of Canada. The facility at 346 Platt's Lane has been in operation since 1985. The St. Joseph's Family Medical and Dental Centre provides comprehensive health care with a patient-centered focus. Family medicine is delivered through active health promotion, ongoing supportive care and the active treatment of disease. We have expanded our primary care support to include a trauma-informed care approach to individuals with complex physical and emotional needs, supporting their journey to healing and recovery.
The Family Medical and Dental Centre continues to be a partnership between St. Joseph's Health Care London and the University of Western Ontario Schulich School of Medicine and Dentistry. The centre supports a quality undergraduate and post-graduate education program, advancing medical knowledge through clinic research.
The Family and Medical Centre is seeking a dedicated and skilled Registered Practical Nurse to join our team. The RPN will play an integral role in the interdisciplinary care team, delivering compassionate, trauma informed nursing support to patients. Your expertise and empathy will help foster a safe and supportive environment for patients with diverse and often complex medical and emotional challenges. This position will focus on providing assessment, screening, healthy lifestyle support, education and chronic disease management from a trauma-informed lens with a goal of improving health outcomes.
Essential Qualifications
- Valid registration with the College of Nurses of Ontario as a Registered Practical Nurse.
- Valid CPR certificate
- Proficiency and experience in pharmacology and medication administration
- Successful completion of a nursing competency test
- A minimum of 2 years of nursing experience in primary care within the last 3 years
- Demonstrated knowledge of trauma-and-violence-informed principles and practices.
- Demonstrated knowledge of family practice nursing throughout an individual's life cycle, along with comprehensive assessment skills
- Familiarity with harm reduction, anti-oppressive frameworks, and culturally safe care.
- Proficiency with electronic medical records (EMR) and other digital tools.
- Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
- Demonstrated experience in Care Coordination and System Navigation to support patients care closest to their home
- Demonstrated competency in phlebotomy skills
- Previous experience dealing with health promotion and preventative medicine
- Demonstrated effective communication, collaborative and interpersonal skills
- Demonstrated expertise in critical thinking and clinical problem-solving skills
- Self-directed, strong organizational skills and ability to prioritize workload
- Ability to work independently as well as on a team
- Demonstrated computer experience, particularly with Outlook, Excel, Microsoft Office Products, particularly PowerPoint and MS Teams
- Ability to apply effective body mechanics and meet the physical demands of the position
Preferred Qualifications- Experience working in trauma-and violence-focused clinics, mental health care or mental health nursing
- Certification or training in trauma-and-violence-informed care or mental health nursing
- Experience with diverse and underserved populations
- Proficiency in French is considered an asset
Teaching and Research- St. Joseph's Health Care London through its affiliation with Western University and Fanshawe College is a leading research and teaching hospital. As an employee of St. Joseph's, you will be expected to engage in role related teaching and research activities in addition to any of your clinical duties
Immunization Requirements- Provide vaccination records or proof of immunity against measles, mumps, rubella, varicella (chicken pox), Hepatitis B, COVID-19 and influenza.
- Provide documentation of the Tuberculosis skin testing
