Salary Range: $69.46 - $81.71 /hour
Specialized Geriatric Services (SGS) provides specialized care to older adults living with frailty and their caregivers in a variety of care settings. We are also the host organization for the Regional Geriatric Program of Southwestern Ontario and actively participate in provincial and regional initiatives aimed at improving health outcomes and the experience of older adults. In addition to providing direct care to older adults and their caregivers, St Joseph's Health Care London is also actively involved with the education of future health providers and in advancing research. Engaging patients and families as full partners in the care we provide is a strategic priority across the organization.
We are seeking a Nurse Practitioner/Clinical Nurse Specialist to work within a new and unique clinical pathway, whereby specialized geriatric services are provided in Ingersoll Hospital, Oxford County. This clinic aims to serve older adults and their caregivers in a clinic location close to home. The successful candidate will provide direct, specialized, comprehensive care, and provide clinical consultation and leadership within the Ambulatory Geriatric program. They will also participate in the education of staff and clinical partners in the clinical specialty of SGS and incorporate best practice and research findings to promote quality patient/family care. The successful candidate to this position will forecast and anticipate future trends and work collaboratively with leaders to influence policy, implement new standards and regulations, and participate in quality improvement initiatives to enhance care delivery to older adults within our community.
Essential Qualifications
- Valid registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class with specialty certificate in either Primary Health Care or Adult with no restrictions
- Master's of Science in Nursing (MScN) or Master of Nursing (MN) or health care related master's degree
- Member of RNAO, NPAO, Canadian Nurses Protective Society (CNPS) with professional liability insurance
- Minimum three years recent clinical experience in delivering geriatric care in a community and/or outpatient setting
- Superior interpersonal, coaching and communication skills to effectively work within an inter-professional ambulatory team setting
- Demonstrated critical thinking and problem-solving skills
- Collaborate with the inter-professional team to develop, implement and evaluate innovative patient/family centred care delivery practices
- Promotes evidence-based practice
- Previous teaching experience and proven ability to provide clinical leadership
- Experience and demonstrated skills in implementing quality improvement initiatives and leading change initiatives
- Promote the values of shared leadership and inter-professional team collaboration
- Demonstrated experience in project management, specifically related to nursing education and quality improvement
- Participate and conduct research related to the Frail Seniors patient population
- Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
Preferred Qualifications- Consideration may be given to a candidate who is in the process of completion of a Primary Care or Adult Nurse Practitioner program
- Specialty certificate in either Primary Health Care (NP-PHC) or Nurse
- Advanced knowledge and skills with a multi system focus on physical assessment, along with appropriate diagnostic and therapeutic considerations, consultative support and evaluation specific to the care of older adults with complex medical and functional conditions

Immunization Requirements- Provide vaccination records or proof of immunity against measles, mumps rubella, varicella (chicken pox), Hepatitis B, COVID-19 and influenza.
