Salary Range: $40.36 - $56.30 /hour
The Complex Care Program provides care to patients who require ongoing medical management as well as rehabilitation due to a major medical event, accident or illness. The program provides slow stream rehabilitation for individuals with complex and chronic medical conditions. Patients are admitted with the goal to transition to the community within three months.
The Complex Care Program also supports individuals who require specialized continuing care or palliative care. This includes patients requiring long-term mechanical ventilation. Individuals in this program receive residential care in a more appropriate setting to care for and manage their health, welfare and social needs. . Palliative care provides pain and symptom management and end of life care for patients with a life-limiting illness. Our interdisciplinary team provides a holistic and collaborative approach to care.
As a member of the Complex Care inter-professional team, you will demonstrate expertise in providing physiotherapy care and services, including assessment, treatment, discharge planning and community liaison for adult patients with complex, chronic illnesses,
Essential Qualifications
- Current registration with the College of Physiotherapists of Ontario
- Minimum 2 years physiotherapy related experience in musculoskeletal or neurological rehabilitation
- Experience and demonstrated competency in establishing rehabilitation goals and evidence-based treatment plans
- Understanding of complex medical conditions and related co morbidities
- Understanding of palliative and end of life care
- Demonstrated knowledge base in cardiorespiratory physiotherapy
- Familiarity with outcome measurement related to clinical practice
- Knowledge of community resources for patients being discharged.
- Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
- Demonstrated experience using a patient centered approach
- Capable of working independently as well as collaboratively within an interdisciplinary team setting
- Excellent interpersonal, organizational and strong communication skills, both written and verbal
- Proven flexibility and a high level of initiative
- Familiarity with outcome measurement related to clinical practice
- Demonstrated ability to incorporate evidence-based research into practice
- Knowledge of computer systems (e.g. workload measurement, Cerner, Outlook, Microsoft Office)
Preferred Qualifications- ADP authorizer
- Membership in the Canadian Physiotherapy Association and relevant interest groups
- Working knowledge and experience with theories and principles of guiding adult learning and patient self-management approaches
- Demonstrates initiatives with program development/improvement
- Knowledge of wound care

Immunization Requirements- Provide vaccination records or proof of immunity against measles, mumps rubella, varicella (chicken pox), Hepatitis B, COVID-19 and influenza.
- Provide documentation of the Tuberculosis skin testing
