The St. Joseph's Health Care London Primary Care Diabetes Support Program brings together comprehensive diabetes management and leading-edge care and research. While our researchers focus on innovation and discovery, our interdisciplinary team of care providers and educators work to support our patients' goals and health care needs, providing a continuum of services to ensure optimal health and quality of life. Serving southwestern Ontario, the Primary Care Diabetes Support Program is affiliated with Western University, the Lawson Health Research Institute and the Center for Studies in Family Medicine.
The Primary Care Diabetes Support Program has been designed to support the diabetes care needs of community based individuals, living with diabetes, who are experiencing complex self-care challenges related to poverty, mental illness or addictions and the comprehensive health care needs of individuals who have diabetes and are without a Family Physician.
The Primary Care Diabetes Support Program is located at the St. Joseph?s Family Medical Centre and is offering:
- Comprehensive health care for those individuals with diabetes without family physicians
- Diabetes case management, in partnership with primary care providers, for individuals experiencing complex self-care challenges
- Outreach to existing points of contact to engage hard to reach sectors of the local community
The Registered Dietitian (Certified Diabetes Educator) works collaboratively with the interprofessional team to provide client-centred evidenced-based and coordinated diabetes care, including: health monitoring, clinical examination for diabetes-related complications, education and insulin adjustment to individuals with diabetes who may or may not have a primary healthcare professional and who may have co-morbidities affecting their ability for self-management. The dietitian will use skills in individual and group-based nutrition education to facilitate the provision of client-focused diabetes care.
Essential Qualifications
- Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
- Current registration with the College of Dietitians of Ontario
- Eligible for membership with Dietitians of Canada
- Certified Diabetes Educator
- Demonstrated knowledge of current evidence-based practice related to diabetes, including pathophysiology, nutritional strategies, and self-care management
- Demonstrated expertise in developing and implementing nutritional care plans
- Knowledge of community resources and the ability to assist patients and families to navigate the health care system.
- Recent experience in providing nutritional care and diabetes education in a primary care
- Knowledge of education theory (including adult learning principles) and demonstrated experience in the development of diabetic teaching materials and the teaching and education of patients (and family/caregivers) and peers
- Previous experience dealing with health promotion and illness prevention
- Demonstrated effective communication, collaborative and interpersonal skills
- Demonstrated expertise in critical thinking and clinical problem-solving skills
- Self-directed, strong organizational skills and ability to prioritize workload
- Ability to successfully complete the Insulin Adjustment Certification Examination within six months
Preferred Qualifications- Recent experience in providing nutritional care and diabetes management in a mental health context
- Proficiency in French would be an asset.
- Demonstrated insulin pump education, experience and certification
- Experience in public speaking and preparation of educational material
- Demonstrated computer experience, particularly with Outlook, computerized patient information systems, Microsoft Office products (specifically PowerPoint and MS Teams)
Teaching and Research- St. Joseph's Health Care London through its affiliation with Western University and Fanshawe College is a leading research and teaching hospital. As an employee of St. Joseph's you will be expected to engage in role related teaching and research activities in addition to any of your clinical duties
Immunization Requirements- Provide documentation you have received two doses of the COVID-19 vaccine (primary series, boosters and/or XBB) OR one dose of XBB vaccine at least 14 days prior to the start date.
- Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox)
- Provide documentation of the Tuberculosis skin testing
