A challenging opportunity presently exists for a highly motivated, self directed and innovative nurse educator as part of our team with The Palliative Pain and Symptom Management Consultation Program (PPSMCP) of Southwestern Ontario. The PPSMCP requires a Nurse Educator to plan, coordinate and implement the delivery of the PPSMCP education and consultation services across the South West and Erie- St. Clair region. The primary focus would be supporting the Oxford- Elgin areas or as the need arises within the program.

The position may require some travel.

This position will provide palliative care education, consultation and system linkages to enhance the quality of palliative care services, and will function in an organized and integrated manner within the PPSMCP team and the Erie- St. Clair and South West region, consistent with the standards and norms outlined by the Canadian Hospice Palliative Care Association (CHPCA 2002).

Essential Qualifications

  • Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
  • Current certificate of competence with the College of Nurses of Ontario
  • Bachelor of Science in Nursing
  • Certified in Hospice Palliative Care Nursing Canada [CHPCN(C)], and ongoing maintenance
  • A minimum of three years experience in hospice palliative care
  • Demonstrated clinical expertise in all the Hospice Palliative Care Domains of Issues Associated with Illness and Bereavement, as outlined by CHPCA. This expertise must be at an expert level
  • Demonstrated expertise in mentorship, coaching and clinical consultation in individual and group settings
  • Demonstrated understanding of, and relationships within the hospice palliative care system.
  • Successfully completed and/or facilitated the Fundamentals of Hospice Palliative Care
  • Successfully completed and/or facilitated the Comprehensive Advanced Palliative Care Education (CAPCE)
  • Demonstrated experience with curriculum planning and development
  • Demonstrated knowledge and experience applying adult learning theories and principles
  • Demonstrated expert level group facilitation and group motivational skills
  • Demonstrated expertise in program planning and implementation
  • Demonstrated ability to be self directed, and manage multiple projects
  • Demonstrated communication, interpersonal, organizational, problem solving and innovation skills
  • Proficiency in Microsoft applications
  • Access to and demonstrated ability using the internet
  • Ability to travel throughout primarily the Erie St. Clair and Southwest as required, with the need for flexibility for some work into the evening and/or weekends, based on the needs of the program.
Preferred Qualifications
  • Proficiency in French would be an asset
  • Experience in all healthcare sectors (hospital, community, hospice and long term care)
  • Knowledge of program evaluation and outcome measurement
  • Certificate or Diploma in Adult Learning

Immunization Requirements
  • Provide documentation you have received two doses of the Covid-19 vaccine and a written commitment to follow any future Public Health recommendations and St. Joseph's Health Care London policies and processes related to Covid management in the workplace.
  • Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox)
  • Provide documentation of the Tuberculosis skin testing
