A challenging and exciting opportunity exists to work in a collaborative inter-professional environment delivering services to rehabilitation patients in a specialized rehabilitation setting. Using the principles of patient centred care, the Social Worker is a member of an interdisciplinary team, who will be required to provide assessment, recommendations, consultation, counseling, discharge planning and educational services to individuals recovering from stroke/neurological conditions, acquired brain injury and spinal cord injury, along with their care providers. The current assignment for this position will be with the Community Stroke Rehabilitation Team (CSRT) which would include primarily working in the community as part of the CSRT in the London and surrounding area.



Essential Qualifications

  • Valid registration with the Ontario College of Social Workers and Social Service Workers
  • Minimum five years of clinical experience in a health care setting
  • Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
  • Experience and knowledge of stroke and/or neurological conditions
  • Highly developed problem-solving skills as well as the ability to function effectively independently and within a multi-disciplinary team
  • Excellent interpersonal, organizational, time management skills
  • Demonstrated experience and competence in delivering the full range of clinical social work services with an outcome-oriented approach
  • Demonstrated strength in written/verbal communication skills
  • Strong psychosocial assessment and therapeutic intervention skills related to adjustment to chronic illness or disability
  • Strong discharge planning skills with comprehensive knowledge of and ability to mobilize community resources
  • Ability to accept and positively influence change in the work environment
  • Demonstrated skills in providing education to patients, families and the health care team
  • Strong crisis intervention, advocacy, conflict management and facilitation skills
  • Familiarity with outcome measurement related to clinical practice, ability to measure and evaluate practice change outcomes
  • Proficient computer skills with a variety of programs
  • Appreciation of resource utilization and comfortable working in a virtual environment
  • Travel within the catchment area is required
Preferred Qualifications
  • Master of Social Work
  • Proficiency in French would be an asset

Immunization Requirements
  • Provide documentation you have received two doses of the Covid-19 vaccine and a written commitment to follow any future Public Health recommendations and St. Joseph's Health Care London policies and processes related to Covid management in the workplace.
  • Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox)
  • Provide documentation of the Tuberculosis skin testing
