Southwest Centre for Forensic Mental Health offers an exciting opportunity for a licensed Pharmacist to practice pharmaceutical care. You will be responsible for identifying and resolving drug related problems for individuals admitted to a medium security Forensic Mental Health facility. As a valued member of the interdisciplinary team, you will interact with physicians and allied health professionals to optimize drug therapy in a patient-focused environment.

You will be a drug information resource and provide input on drug formulary and drug utilization issues. You will be involved in admissions and discharges as well as order verification. Opportunities for involvement include developing clinical practice guidelines, professional education and student education.

Essential Qualifications

  • Part A licensed pharmacist in good standing with the Ontario College of Pharmacists
  • Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
  • Ability and commitment to act as a supervisor under OHSA
Preferred Qualifications
  • Experience with compounding Sterile and Non-Sterile products
  • Graduate of an accredited Hospital Pharmacy Residency program
  • Recent Hospital experience
  • Proficiency in French language is considered an asset
Teaching and Research
  • St. Joseph's Health Care London through its affiliation with Western University and Fanshawe College is a leading research and teaching hospital. As an employee of St. Joseph's you will be expected to engage in role related teaching and research activities in addition to any of your clinical duties
Immunization Requirements
  • Provide documentation you have received two doses of the Covid-19 vaccine and a written commitment to follow any future Public Health recommendations and St. Joseph's Health Care London policies and processes related to Covid management in the workplace
  • Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox)
  • Provide documentation of the Tuberculosis skin testing
