The Veterans Care Program assists war service Veterans who are unable to live at home, whose daily functioning and health are affected by chronic physical and mental illnesses and episodic health problems, in an in-patient setting. As a member of the interprofessional team, you will promote the health and well-being and meet the social, emotional and spiritual needs of the Veterans. The Veterans Care Program supports Veterans to be as independent as possible and to reach their full capability within a safe environment. Families are key partners in our care team and are supported by the team to adjust and adapt to the changing needs of the family member.

As a Social Worker, you will provide a full range of clinical social work interventions as appropriate to the needs of the veterans and their families / significant others, including psychosocial assessment, supportive counseling, and psychotherapy, consultation and education within an intergenerational context.



Essential Qualifications

  • Valid registration with the Ontario College of Social Workers and Social Service Workers
  • Minimum 2 years' experience in social work practice in hospital or long-term care, with expertise in the care of the elderly including dementia, psychogeriatrics and complex and chronic health conditions
  • Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
  • Demonstrated experience and competence in delivering the full range of clinical social work services with an outcome-oriented approach
  • Demonstrated competence in counseling and psychotherapeutic modalities with the elderly, and their families / significant others, including the areas of grief, bereavement, depression and mental illness, intergenerational issues, caregiver stress, adjustment to illness, hospitalization and long-term care
  • Ability to execute early problem and needs identification through case finding, assessment and implementation of social work interventions in collaboration with the patient, family and interprofessional team
  • Strong discharge planning skills
  • Comprehensive knowledge of and ability to mobilize community resources
  • Demonstrated strength in written/verbal communication and case management
  • Strong crisis intervention, advocacy, conflict management and facilitation skills
  • Highly developed interpersonal and problem-solving skills as well as the ability to function effectively independently and within a multi-disciplinary team
  • Excellent organizational, time management and communication
  • Demonstrated skills in providing education to patients, families and the health care team
  • Supports research and innovation in practice, previous experience in research an asset
  • Proficient computer skills with a variety of programs
  • Knowledge of relevant legislation including the Health Care Consent Act and Substitute Decisions Act
  • Ability to accept and positively influence change in the work environment
  • Familiarity with outcome measurement related to clinical practice, ability to measure and evaluate practice change outcomes
Preferred Qualifications
  • Proficiency in French would be an asset
  • Masters Degree in Social Work
  • Experience working with war service veterans and their families
  • Expertise in addictions
  • Membership with the Ontario Association of Social Workers

Immunization Requirements
  • Provide vaccination records or proof of immunity against measles, mumps, rubella and varicella (chicken pox)
  • Provide documentation of the Tuberculosis skin testing
