Social Worker (BSW) - 1 position
NeuroTrauma Rehabilitation (NTR)

Full Time

As a member of the interdisciplinary teams, the Social Worker would be required to provide assessment and rehabilitation services to clients 16 years and over who have experienced a neurological trauma from accident or disease, including people with acquired brain injury, spinal cord injury, multiple trauma, and complex orthopedic injuries. Additionally, the Social Worker will provide counseling and education to patients, family members and service providers in the community. These are community based programs assisting clients in such settings as home, school, work, etc.

Essential Qualifications

  • Valid registration with the Ontario College of Social Workers and Social Service Workers
  • Minimum 2 years of social work experience
  • Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
  • Demonstrated experience and competence in delivering the full range of clinical social work services with an outcome-oriented approach
  • Knowledge of Mental Health Act, Health Care Consent Act, PHIPA and other relevant legislation
  • Clinical experience in rehabilitation principles and practices related to neurological trauma
  • Experience with family counseling
  • Demonstrated ability to assess physical and psychological needs of patients
  • Demonstrated expertise in treatment planning
  • In-depth knowledge of major mental illness from a biopsychosocial perspective
  • Ability to execute early problem and needs identification through professional assessments and skillful implementation of all planned social work interventions in cooperation with the patient, family and interdisciplinary team
  • Strong crisis intervention, advocacy, conflict management and facilitation skills
  • Demonstrated skills in providing education to patients, families, the health care team and the community
  • Knowledge of community health service resources, knowledge of WSIB and motor vehicle insurance and related issues
  • Highly developed interpersonal and problem-solving skills as well as the ability to function effectively independently and within a multi-disciplinary team
  • Excellent organizational, time management and communication
  • Supports research and innovation in practice, previous experience in research an asset
  • Strong discharge planning skills
  • Comprehensive knowledge of and ability to mobilize community resources
  • Ability to accept and positively influence change in the work environment
  • Demonstrated strength in written/verbal communication and case management
  • Demonstrated skills in providing education to patients, families and the health care team
  • Proficient computer skills with a variety of programs
  • Familiarity with outcome measurement related to clinical practice, ability to measure and evaluate practice change outcomes
  • Ability to work independently and collaboratively with a wide range of health care professionals, with case managers, insurance companies and attorneys
  • Travel within the catchment area
Preferred Qualifications
  • Proficiency in French would be an asset
  • Masters of Social Work
  • Minimum five years of clinical experience

Immunization Requirements
  • Provide vaccination records or proof of immunity against measles, mumps rubella and varicella (chicken pox)
  • Provide documentation of the Tuberculosis skin testing
